Health and wellness EXAMS


Wellness exams are a covered health care benefit that may lower your risk of illness. During your annual wellness visit our team, with your input, will develop a personalized plan to reduce your health risks. We’ll ask questions about your health and you will need to complete new history forms along with some other paperwork. Have your reading glasses handy – if you need them, or complete the wellness forms ahead of time and bring to you appointment!

  • Screenings to detect depression, risk for falling, and other problems
  • Limited physical exam includes blood pressure, weight, vision
  • Discussion about healthy lifestyle
  • Recommendations for additional services base on age, gender, activity level and your family history

Your insurance pays for some wellness care, but it may not pay for all the care you need under the wellness benefit. Recommendations that are suitable just for you may be made and will be performed once given your permission. 

A wellness visit does NOT include discussing new or existing health problems – that is a separate appointment. A separate charge applies to these services, whether provided on the same date or a different date than the wellness visit. Please tell our staff ahead of time if you need the doctor’s help with a health problem, a medication refill of something else as we may need to schedule a separate appointment.
